EMT Renewal Application Process

As the EMT-B license expires every three years, the EMT will need to fill out the Application Packet for Initial Certification & Recertification of EMS Providers to maintain your license with WA state. To practice as an EMT you must be licensed, and to be licensed you must have a sponsoring agency, in this case, the King County Sheriff's Office (KCSO).

Application Packet for Initial Certification & Recertification of EMS Providers:

Online: https://fortress.wa.gov/doh/emsonline/EMSLogon.aspx
You will need to know your EMS Registry Number (EMT license number) and the last four digits of your Social Security Number. Once you have logged in and applied for recertification on your profile this request will be submitted for the KCSO SAR Coordinator to approve your recertification application online.

Print it out and mail the original to the KCSO SAR Coordinator for approval, proper signatures, and to be forwarded to KC EMS.

Personal Status Change: This form is used to add another sponsoring agency or to update contact information.

You will need to know your EMS Registry Number (EMT license number) and the last four digits of your Social Security Number. Once you have logged in, make the changes to your profile and they will be submitted for the KCSO SAR Coordinator to approve your status change online.
